instant death never takes modified toughness into account unless specifically stated.
as for looking for a definitive answer to your stacking question in the codex, i doubt you will find one, but trust me it does stack.
my best example of this is unfortunately uses strength rather than toughness. an Ork warlord (S5) has a big choppa (+2S) and in the turn he charges gains Furious charge (+1S)
there is no rule to suggest that the strength bonuses do not stack, nor is their one to suggest that toughness bonuses do not stack (apart from in the case of instant death as pointed out). |
